The death toll from Friday morning's accident outside Uitenhage has risen to four.
Police spokesperson, Priscilla Naidu, says an Eastern Cape Health Department worker who was injured in the accident died in hospital.
Earlier, she said three people were killed when a mini-bus taxi collided with a state pathology vehicle on the road between Rocklands and Uitenhage. Three others were seriously injured.
The names of the deceased have not yet been released.
Meanwhile, Eastern Cape police have expressed concern at the increasing number of deaths as a result of drowning.
This, as the number of drownings in December climbed to 45. The victims include children, circumcision initiates and adults.
According to a statement from provincial police, children who go swimming should be accompanied by someone who can swim.
Police said alcohol also played a major role in many drowning incidents.
They've advised people to swim in areas where there are designated lifeguards.
