A man and a woman were killed instantly late on Wednesday afternoon in an accident on the R335 between Addo and Kirkwood.
Provincial Traffic Deputy Director Charles Bramwell says the couple's car collided with a large horse and trailer truck.
According to the truck driver, the car skidded across the road and went under his vehicle. It was dragged for more than 100 metres before coming to a standstill.
Bramwell says rescue teams had to use the jaws of life to remove the two bodies from the wreckage.
The truck driver was treated for slight injuries.
The victims were 54-year-old Simphiwe Mente and 25-year-old Thandiwe Nolingo who were originally from Addo.
