Three truck drivers were killed and one critically injured after two trucks collided head-on on the N10 near Olifantskop pass on Wednesday.
One of the trucks was ferrying a load of cattle towards Cookhouse and at least 14 cows were killed when they were consumed by a fire caused by the crash.
The person who survived the crash was a co-driver of one of the trucks and was rushed to hospital in Somerset East where he is in critical condition.
Law enforcement and rescue personnel who responded to the scene managed to free some of the cattle, some of which were seriously injured.
According to officials, there will still be delays on the stretch of road on Thursday as crash scene investigators are sifting the scene, and the recovery of the wreckage is set to resume.
The names of the deceased have not been released.
