Three people were killed in a head-on collision on the N2 near Peddie on Saturday.
The Eastern Cape Transport Department says a VW Polo was heading in the direction of Peddie when it collided with an oncoming truck.
The three occupants in the Polo, all women, were declared dead at the scene, while the truck driver escaped unharmed.
The Department says it alleged that driving under the influence of alcohol and reckless and negligent driving were the main contributing factors.
Police are investigating a case of culpable homicide.
