The Eastern Cape Transport Department said seven people were killed in an accident involving a car and a bakkie early on Saturday evening.
Spokesperson, Unathi Binqose, said one person sustained serious injuries in the crash on the R58 between Lady Grey and Maletswai (Aliwal North).
He said a Mazda sedan with three males, two females and minor boy, collided head-on with a Toyota bakkie with two occupants, a man and a woman.
Binqose said all six occupants in the sedan, including the child, died on the scene, while the female passenger in the bakkie also succumbed to her injuries.
The bakkie driver was rushed to a hospital in Maletswai with serious injuries.
A case of culpable homicide has been opened at Lady Grey Saps for further investigation.
