Port Elizabeth police are at the scene of an accident near Coega, where a Transnet vehicle was swept from a low-water bridge.
Spokesperson, Captain André Beetge says four men were on their way to work when the incident happened on Tuesday morning.
He says the river's water level has risen after heavy rain over the weekend.
Beetge says three men were rescued, but a fourth is still missing.
He says it is not certain if the missing man was the driver.
