Six people, including five children, died in a head-on collision on the R61 road near Mount Nicholas in Libode on Monday.
The Eastern Cape Transport Department said a sedan collided with a fully loaded minibus taxi that was travelling from Mthatha.
A spokesperson said an adult and five children in the car were declared dead on the scene.
14 people from the taxi sustained injuries and were rushed to St Barnabas Hospital for further medical care.
A case of culpable homicide has been registered with the SAPS at Libode.
The latest fatalities bring to at least 28 the number of people killed on the province's roads this past weekend.
