The weather office in Port Elizabeth says temperatures in and around the city have hit 33 degrees Celsius with a discomfort index of 101.
Spokesperson Gart Sampson says the level is still rising and is expected to hit 105 plus.
Schools in Port Elizabeth and Uitenhage are advised to cancel outdoor sports.
He says all strenuous outdoor activities should be ceased.
Sampson says people are advised to stay indoors and stay hydrated.
The old and infirm are also adviced to stay indoors.
