Rescue workers and police are on the scene at the notorious Voëlklip Lookout point in Herlods Bay, outside George where a car plunged over the cliff.
According to reports a mother and two children were in the vehicle.
Police spokesperson Captain Malcolm Pojie says one of the children, an 8-year-old boy, managed to escape before the car went over.
He says the mother and her two-year-old daughter are still missing.
Captain Pojie says the woman was driving a white Fiat.
Rescue workers and a helicopter have been dispatched to look for any survivors.

The last incident at Voëlklip was on 31 January 2020 when 'n 52-year-old man from Port Elizabeth was seen driving his vehicle off the cliff.
In October 2019, the 35-year-old Heidi Scheepers and her two children went missing and their vehicle was found at the bottom of a cliff when a search party noticed tyre tracks near a ledge at Voëlklip.
The body of her son, Hugo was found first and Heidi's body was retrieved from the water a few days later.
A post mortem concluded that they both drowned.
The body of her six-year-old daughter Cozette was never found.
