An ambulance and truck driver were killed following a head-on collision on the N10 road in Nxuba (formerly Cradock) about 45 km towards Cookhouse on Wednesday.
Provincial spokesperson Unathi Binqose says an articulated truck collided with the ambulance shortly after 7 am.
He says both drivers died on the scene.
The truck driver was alone in his vehicle, while there was one crew member in the ambulance who sustained serious injuries.
Binqose says the injured person was taken to the Cradock Provincial Hospital.
A case of culpable homicide is under investigation.
The names of the deceased cannot yet be released.
