According to police two vehicles and a bakkie were involved in the incident that happened late Thursday afternoon.
Seven passengers sustained minor to moderate injuries.
Both lanes in the Outeniqua Pass had to be closed while paramedics were on the scene.
Police have opened a culpable homicide docket as the cause of the accident is still being investigated.
The names of the deceased have not yet been released.
