Three people died and seven others were injured in a head-on collision on the R 75 near Uitenhage on Sunday night.
Netcare 911's Chris Botha says paramedics arrived at the scene and found both lanes of the roadway obstructed with accident debris and the twisted remains of the vehicles.
Some of the injured were still trapped in the wreckage.
Botha says rescue crews needed to use the jaws of life to remove the bodies of two people, who had died at the scene, and eight other seriously injured people.
After the patients were stabilised on the scene, paramedics transported them to various hospitals in the area.
Botha says one of the critically injured crash victims died soon after arriving at hospital.
