Western Cape authorities reported the deaths of four people in a head-on collision between a car and a truck carrying livestock on Wednesday.
Police spokesperson, Sergeant Chris Spies, said the horror accident happened on the N2 near Heidelberg in the afternoon, about 10 km from the town.
He said all the occupants in an Audi A4 died on the scene.
Spies said the truck driver sustained multiple injuries and was admitted to a nearby hospital.
The identities of the deceased have not yet been released.
Heidelberg police have opened a culpable homicide docket.
