Five men were killed when the vehicle they were travelling in overturned on the N2 road near Betshwana in Mount Ayliff.
A spokesperson for the Department of Transport Unathi Bingqose says only one person survived the accident, which took place just after 6 am on Monday.
He says it is not yet clear what lead to the accident and it will form part of the culpable homicide investigation.
The victims, including the driver, were aged between 25 and 40.
Binqose says the man who survived the accident sustained serious injuries and was taken to a nearby hospital.
He says it is believed that the Nissan NP200 LMV was travelling from Kokstad towards Mt Ayliff when it lost control.
