The death toll in Monday nights accident on the R58 between Aliwal North and Lady Grey has risen to eight.
A spokesperson for Provincial Traffic Unathi Bhingqose says two of the three people who were hospitalised with injuries, died.
They were killed as the result of a head-on collision, involving a Kia Sedan and Mazda.
The drivers of both vehicles died on the scene as well as another man, woman and a child.
The names of the deceased have not yet been released.
Bhinqose says the accident could have been avoided if one of the drivers had just been patient.
