Four people have died while nine others sustained critical injuries after the Toyota quantum they were driving in lost control and crashed on the R61 between Graaff-Reinet and Cradock.
The accident happened at 6.30 am on Friday about 20 km outside Cradock.
Department of Transport spokesperson, Khuselwa Rantjie, said the vehicle was travelling from Cape Town with its final destination expected to be Ntabethemba.
It is alleged that the driver lost control of the van at a sharp curve causing it to swerve off the road killing three women and a man.
Rantjie said nine others including the driver, sustained serious injuries, while two others escaped with only slight injuries.
She said the injured passengers were taken to the Midland Hospital in Graaff-Reinet for further medical treatment.
Rantjie further said that speed and fatigue may have contributed to the fatal crash.
