A Gqeberha man accused of murdering a patient at the Elizabeth Donkin Psychiatric Hospital appeared in the Magistrates Court on Friday.
The murder is said to have taken place on 28 January.
Erin-Jade Fillis, 21, allegedly killed his roommate, Gradwell Jordaan, 36, by slamming his head into a brick wall.
According to police, a fight had erupted between the two.
On Friday, the court heard that Fillis had consulted with the District Surgeon, who recommended he be sent for a psychiatric evaluation.
According to the District Surgeon's report, the accused had admitted to "killing someone."
The report further describes him as restless and avoidant, mentally unstable, and states that he appeared to show no remorse.
Fillis has also admitted to using drugs like dagga, Mandrax and Tik.
The case was remanded to 8 March.
