Two people were killed in head-on collision between two cars at an intersection in Alberton on Saturday morning.
ER 24 said one of the vehicles also caught alight.
Spokesperson, Ineke van Huysteen, said when their paramedics arrived on the scene they found that one car had completely burned out while the other car was on the side of the road.
She said paramedics from another medical service was attending to one of the injured motorists who was in critical condition.
"Despite paramedics' best efforts the patient succumbed to his multiple injuries and was declared dead at the scene," she said.
Van Huysteen said that the other person had burnt to death in the other car.
Police are investigating cases of culpable homicide.
