Six people were killed following a head-on collision on the N12 outside Potchefstroom on Sunday evening.
A spokesperson for ER 24 Russel Meiring, says when paramedics arrived at the scene, about 20 km outside of Potch, they found the two cars on opposite sides of the road.
He says one patient was from one of the vehicles while seven others were found lying around the other vehicle.
Meiring says six people, including a toddler, was declared dead on the scene.
Two people sustained moderate to serious injuries and were transported to a hospital.
A case of culpable homicide is now under investigation.
