Seven people, including a one-year-old, were killed and four others were injured when a Toyota Fortuner and a truck collided in Bloemfontein on Sunday.
ER24's Werner Vermaak says the Fortuner was ripped apart, and the truck overturned on the N1 - south towards Cape Town.
He says it appears that some of the occupants from the Fortuner were flung from the vehicle by the impact.
Vermaak says three occupants of the car were rushed to nearby hospitals.
The cause of the accident is not known and local authorities are investigating.
